This is the paperwork which should be completed and emailed to stpatricks@secularclergy.org.uk to confirm the burial booking within 48 hours of the booking being made.

We now charge for historical searches. Please download this form and, once you have completed it, please submit it by email to stpatricks@secularclergy.org.uk.
